Abstract

Background: Physiotherapeutic interventions are often employed in the thoracic region. To date the segmental effect on the sympathetic nervous system after activation of skin and muscles is unclear. Objective: The aim of this study was to evaluate (1) the feasibility and (2) the segmental perfusion and erythemal response in the thoracic spine segment following massage, mobilization, manipulation and placebo therapy in order to develop a study design and to establish a first possible hypothesis for perfusion and erythema changes. Method: In this single case study a healthy subject received 4 different treatment interventions on 4 days. Massage, mobilization, manipulation and additionally placebo intervention were applied in the area between th 3 and th 6. Blood perfusion and skin erythema in the related dermatome were measured before, during and after the intervention for 55 minutes at time intervals of 5 minutes. Results: Manipulation and mobilization resulted in an acute increase of the sympathetic nerve tone whereas massage and placebo treatment resulted in a decrease due to a change of perfusion microcirculation. Conclusions: For future studies the study design must be adapted with a view towards blinding of therapists and test intervals. Establishing a hypothesis is dispensed because the assessment tool provided no significant data.
